The name is Takotsubo Cardiomyopathy or Broken Heart Syndrome..

Takotsubo is a transient cardiac syndrome that involves left ventricular apical akinesis and mimics acute coronary syndrome (ACS).

MIMICS ACS?

Yes!

The sign and symptoms are similar, that’s why Takotsubo Cardiomyopathy may be misdiagnosed as a heart attack (especially ACS)

But as a medical doctor we must consider the worst possible (ACS)

The most common presenting symptoms are chest pain and dyspnea, though palpitations, nausea, vomiting, syncope, and, rarely, cardiogenic shock have been reported…

The difference Takotsubo and ACS is its association with a preceding emotionally or physically stressful trigger event, occurring in approximately two thirds of patients…

Unlike ACS, for which the peak occurrence is during the morning hours, TCM events are most prevalent in the afternoon, when stressful triggers are more likely to take place..

In 90% patients with Takotsubo, cardiac markers (specifically Troponin I & Troponin T) are elevated and on ECG patients with Takotsubo performing ST-Segment elevation, makes Takotsubo really similar with ACS..

The diagnosis of TCM is typically confirmed in the cardiac catheterization laboratory. In a review of 240 patients diagnosed with TCM, 211 were found to have completely normal coronary arteries, whereas the remainder had noncritical stenoses…

Women are more likely than men to experience the sudden, intense chest pain — the reaction to a surge of stress hormones — that can be caused by an emotionally stressful event. It could be the death of a loved one or even a divorce, breakup or physical separation, BETRAYAL OR ROMANTIC REJECTION. It could even happen after a good shock (like winning the lottery)…

The exact etiology of TCM is still unknown, but several theories have been proposed and are being investigated. These include the following:

• Multivessel coronary artery spasm

• Impaired cardiac microvascular function
